How to Relinquish My Dental License

If you wish to relinquish (give up) your licence, you can access the relinquishment form on the NSRDDA Alinity Registrant Portal.

The process is quite simple. You will be asked to indicate:

• • the effective date of relinquishment, and
  • the reason you are relinquishing (in general terms from a pulldown list).

The process is the same whether you plan to relinquish your licence permanently (e.g., retirement) of for a limited time (e.g., family or health leave).

Future Reinstatement

If you think you may want to reinstate your licence in the future, you should contact our office early to see if there will be any requirements which you will need to fulfill at that time.

Licence Renewal Deadline

If the licence renewal deadline is approaching and you plan to discontinue your licence, it is important that you formally relinquish your licence rather than letting it lapse so that you remain a registrant in good standing.
