How to Relinquish My Dental License

The license year for dentists begins December 1 and expires November 30th.

• You will be invoiced for the annual license fee 30 days prior to license renewal.  Payment of the license fee, and the completed invoice, must be received in the Registrar’s office by November 30th (regardless of date mailed) to avoid cancellation of license.
• If it is your intention not to renew your license, you must advise the Registrar’s office in writing by November 30th to avoid removal of your name from the Register of licensed dentists.
• If it is your intention to relinquish your license at any other time of the year, you must advise the Registrar’s office in writing in order to remain in Good Standing.
• You must also advise your associaiton of your intention.
